Included Coverage for All Full-Time Undergraduate Students

All full-time undergraduate students are automatically enrolled in the at no additional cost. 911��ɱ�� pays the premium for this plan, which covers inpatient, outpatient, and other services outlined by UnitedHealthcare.

Optional Health Insurance Plan (Plan II)

In addition to the injury plan, 911��ɱ�� offers a Student Health Insurance Plan (Plan II) through UnitedHealthcare to help cover more serious or costly illnesses, hospitalizations, surgical care, and physician services.

Annual premium cost: $1,917.00
Coverage is added automatically unless waived by the deadline. (You must waive coverage annually.)

Waiving Coverage

You may opt out of the 911��ɱ�� Health Plan if you have comparable health insurance.

How to Waive Coverage

Visit
Type “911��ɱ��” into the search box and click the magnifying glass.
Select “Waive Coverage” or “Opt-in.”
Choose 911��ɱ�� from the dropdown and click “Continue.”
Click on “911��ɱ�� Waiver Form.”
Enter your date of birth and 911��ɱ�� student ID.
Follow the prompts, complete the form, and click “Yes” if waiving coverage.
Type your name as a digital signature and click “Submit.”
A confirmation screen will appear once the waiver is successfully submitted.

Ensure your current insurance information and 911��ɱ�� student ID are on hand before starting.

Waiver Deadlines

Fall Semester:
- Begin waiving: June 1
- Deadline: September 6
Spring Semester (new students only):
- Deadline: January 15

Students who do not complete the waiver by the deadline will be charged the full annual premium on their tuition and fees bill.
